This form is only valid for Distance Education students and faculty/staff at Shasta College satellite campuses or online courses.
Students taking classes at the main campus must visit the Library and apply in person.
If you are taking classes or working at a Distance Education
site, you may also fill out an application at the Intermountain, Tehama, or Trinity campus or HSUC.
- If you previously had a library card and are requesting a replacement, there is a $2.00 fee.
- You will need to visit the Library on campus or one of the satellite campuses to pay the $2.00 before a new card can be issued.
